How to Register:
IMPORTANT NOTICE-PLEASE READ LIGHTHOUSE SUMMER AND AFTER SCHOOL REGISTRATION 2009-2010
DATE: April, during report card conferences
WHERE: At the school where you want your child to attend
WHEN:At select times (please see the site coordinator for specifics)
Registration is on a first come, first serve basis per grade with half of the available slots reserved for day 1 and the other half for day 2.
SUMMER SCHEDULE AND TIMES: July 1, 2009 through August 7, 2009 from 9:00am to 5:00pm.
EARLY DROP OFF/LATE PICK UP OPTION AND FEE: Drop off at 8:30am-pick up at 5:30pm costs $5.00 per day regardless of scholarship status.
SCHOOL YEAR SCHEDULE AND TIMES: August 31, 2009 through May 28, 2010 from 3:00pm to 5:30pm.
FEE: On a per child basis: $20.00 per week if over 250% poverty income, $10.00 per week if over 225% poverty income, $8.00 per week if over 200% poverty income, $6.00 per week if over 175% poverty income, $5.00 per week if under the 175% poverty income. Scholarships available.
LATE PICK UP OPTION AND FEE: Pick up at 6:00pm for $2.00 per day regardless of scholarship.
ITEMS YOU WILL NEED TO BRING WITH YOU TO REGISTRATION:
• Copy of your Household Income (Copy of your income tax return (1040) for each working parent.Only if you are applying for reduced fee or scholarship status
OR
• Two recent consecutive pay stubs for each working parent, letter showing unemployment status or State budget sheet
• A $25.00 non refundable deposit for the summer program (bank check or money order accepted) No Cash or Personal Checks Please
• A separate $25.00 non refundable deposit for the after school program ( bank check or money order accepted) No Cash or Personal Checks Please
*** Should you be unable to provide “proof of income”, you will be responsible for the full cost.***

PLEASE NOTE: Supplemental tutoring services are available through the Lighthouse Program at select sites beginning in the fall.
